Everything you need before, during and after your visit

Practical information for patients of Millenium Medical Group — how to prepare, what to bring, how billing works and how to request records.

New patients

Bring these to your first visit:

  • Photo ID
  • Insurance card
  • A list of your current medications
  • Recent lab or imaging results, if you have them

Request your appointment

By phone. We confirm a time and tell you what to bring.

Complete registration

Arrive a little early. Registration and consent are handled at the front desk.

Meet your clinician

History, examination and a discussion of your options — with time for questions.

Leave with a plan

A written plan, referrals actioned internally, and a follow-up interval.

Insurance & billing

We work with a broad range of commercial, Medicare and Medicaid plans. Because coverage depends on your specific plan, service and eligibility — and because many services require prior authorisation — we verify benefits with you before your visit.

Accepting a plan is not a guarantee of coverage. Final payment determinations are always made by your insurance plan.

Medical records

You may request a copy of your medical record at any time. Contact our office and we will explain the process, what identification we need and any applicable timelines. Requests are handled under our Notice of Privacy Practices.

Accessibility & language

Services are delivered in English and Spanish, and this website is built to WCAG 2.2 AA intent: semantic structure, keyboard-operable navigation, visible focus states, sufficient colour contrast and text alternatives for meaningful imagery.

If you need an accommodation — a sign language interpreter, materials in an alternative format, or assistance completing paperwork — tell us when you book and we will arrange it.
